48 YEAR OLD MALE WITH ABDOMINAL DISTENSION

  CHIEF COMPLAINTS  Abdominal distension since 20 days  Blood in stools since 3days  Sob since 20 days  Decreased urine output since 3 days  HISTORY OF PRESENTING ILLNESS  Patient was apparently asymptomatic 20 days back when he developed abdominal distension which was insidious in onset ,gradually progressed to current state  ,distension changes w position ,diffuse in type and present throughout the day.  No H/o abd pain obsipation vomiting loose stools constipation Distension is associated with SOB since 10 days initially while squatting currently even at rest(MMRC-grade-1to grade-4)  ,not associated with orthopnea ,PND. . Swelling of lower limbs since 15 days  Insidious in onset  Pitting type ,Initially involving only the feet later progressed Upto knees  No Association with scrotal swelling  No history of chest pain ,palpitations,facial puffiness . history of high coloured urine since 20 days associated with decreased urine output since 10 days  Not associated with burning micturat

I have been given this case to solve in an attempt to understand the topic of " patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with diagnosis and treatment plan.   Chief complaints: Fever since 7 days  Pain abdomen since 7 days  Burning micturition since 3 days History of present illness: Patient was apparently alright 7 days back then he  had fever which is sudden in onset, high grade, with evening rise in temperature associated with chills and rigors, associated with night sweats releived by taking medication. No H/o headache , no H/o body pains, no H/o retro orbital pain pain abdomen since 7 days in right hypochondriac ,right lumbar, umbilical region, needle pricking type of pain , non-radiating and aggrevated during inspiration

65 YEAR OLD MALE PT WITH CHIEF COMPLAINS OF LOSS OF MEMORY

65 yr old male farmer by occupation came with   chief complaints of  Loss of memory since 1 yr  Rt sides upper and lower limb weakness since 1 yr  Burning sensation of both feet since 1 yr  HOPI The patient was apparently asymptomatic 1 yr ago then he developed sudden onset seizures for which he has admitted in the hospital at 10pm. He was unconscious up to 12 hours. after gaining consciousness he happened to observe that the entire right half of the body was feeling weak. His right lower limb was involved more than his right upper limb due to which he was unable to walk.  He also developed deviation of mouth to the left side. he also had an attack of headache, vertigo, and projectile vomiting. No h/o trauma to the head, double vision, or nasal regurgitation.  pt has also developed loss of memory for 1 year which was insidious in onset and gradually progressive the pt was unable to recall the things he has done this morning. No h/o seizure activity again till now  Past illness  K/c/o c
